Primary fusion of the posterior subtalar joint in the treatment of fractures of the calcaneum.

چکیده
The calcaneum is so shaped and so related to the wedge-shaped inferior surface of the talus that when it is fractured the major impact usually falls on the posterior facet of the subtalar joint. This fact is well known. Reference has often been made to it (Malgaigne 1847, B#{246}hler 1935, Watson-Jones 1943) and its importance was again emphasised in the admirable study of fractures of the calcaneum by Essex-Lopresti (1952). The ultimate function of the foot after the calcaneum has been fractured depends mainly upon two factors: 1) what happens to the subtalar joint; and 2) how much secondary stiffness

Early posterior subtalar fusion in the treatment of fractures of the os calcis.
Displaced fractures of the os calcis involving the subtalar joint frequently cause chronic disability due to subsequent osteoarthritis. Early posterior subtalar fusion may prevent this outcome. We have reviewed forty-seven fractures in forty-three patients at an average of seven years after operation. Over 90 per cent of patients had an excellent, good or satisfactory result.
متن کاملSubtalar fusion after displaced intra-articular calcaneal fractures: does initial operative treatment matter? Surgical technique.
BACKGROUND Many patients with displaced intra-articular calcaneal fractures require subtalar arthrodesis for the treatment of posttraumatic arthritis. We hypothesized that patients who underwent initial operative treatment would have better functional outcomes as compared with those who underwent initial nonoperative treatment before undergoing a subtalar arthrodesis. METHODS A consecutive se...
متن کاملFractures of the calcaneum: the anterolateral fragment.
Our previous reports on the pathological anatomy and operative treatment of intra-articular fractures of the calcaneum failed to take account of the fracture pattern anterior to the posterior facet of the subtalar joint. We have reviewed our experience of 63 operative cases and have studied fractures with axial and coronal CT scans reconstructed onto plastic model bones. A constant anterolatera...
متن کاملArthroscopic release for painful subtalar stiffness after intra-articular fractures of the calcaneum.
We describe the surgical technique and results of arthroscopic subtalar release in 17 patients (17 feet) with painful subtalar stiffness following an intra-articular calcaneal fracture of Sanders' type II or III. The mean duration from injury to arthroscopic release was 11.3 months (6.4 to 36) and the mean follow-up after release was 16.8 months (12 to 25). The patient was positioned laterally ...
متن کاملIrreducible posterior subtalar dislocation with incarceration of a fracture of the anterior process of the calcaneum.
Subtalar dislocation is a significant injury characterised by late complications, including subtalar arthritis. We describe a rare case of irreducible posterior subtalar dislocation due to incarceration of a fracture of the anterior process of the calcaneum in the subtalar joint, and discuss appropriate management.
